Although technically it was combining an existing ident with an existing logo the Carlton HTV idents are worth a mention, taking the two generic looks of the time and merging them following Carlton's acquisition of HTV.
Personally I always felt they could have been done much better so they ended with the stars backdrop rather than the GMG Hearts background, but others seem to like them.

And as you mention JAS84 the ITV logo was fairly seemlessly replaced by a bigger ITV1 logo in 2001, but I'd say that's more a resizing of the logo rather than introducing a new logo.

UTV pigeons had the best music out of the lot! Still to this day why only UTV got this music and no other ITV ident used it is a mystery I guess will never be solved.
